Real $407M tenapanor franchise growing 22%, but widening losses, thin cash and XPHOZAH reimbursement erosion cap upside.

Revenue $407M · FY2025

Ardelyx is a two-product commercial-stage biopharma built entirely on tenapanor — IBSRELA (IBS-C) and XPHOZAH (hyperphosphatemia in dialysis CKD). The top line is real and sizable: FY2025 revenue of $407M, up 22.1% YoY, riding a multi-year ramp (FY2021 $10M → FY2024 $334M → FY2025 $407M). That's the bull case: a differentiated, first-in-class NHE3 inhibitor with a sticky IBSRELA growth driver and an early CIC line-extension. At $5.23 the market cap is $1.28B and P/S is 3.1x — not demanding for a 22%-grower if it can convert revenue into profit.

But the quality of that growth is deteriorating, and the balance sheet is the problem. Growth decelerated hard (F) — from triple digits to +22% — while the net loss widened to -$61.6M (worse than FY2024's -$39.1M) and operating margin sits at -10.1%. Cash flow is still negative (operating cash flow -$42.5M) against only $68M of cash. Management is plugging the gap with debt: the June 2025 Fifth Amendment drew $50M of the Term E loan with an option for $100M more, pushing long-term debt to $203M (+34.5%) and liabilities/equity to 2.0x atop a $946.9M accumulated deficit. The 10-K is explicit that cash-flow positivity is not yet achieved and may not be sustained. (The 1.1% 'gross margin' here is computed off a stale FY2019 figure and should be disregarded.)
